YQL enforces an uncompromising stance: any breach of LOTO protocols triggers immediate disciplinary action. Unsafe acts are treated as violations of the Code of Conduct and escalate under HR Policy 8.4.
Supervisor Accountability
Supervisors must stop unsafe work instantly, document violations, and reinforce LOTO discipline during toolbox talks. Leadership accountability is non‑negotiable.
Employee Responsibility
Workers are required to respect locks/tags, follow isolation steps, and report breaches immediately. “No Lock, No Work” is the standard.
